What does an IV therapy medical director do?
An IV therapy medical director is a licensed physician who leads the medical side of your IV clinic. They review treatment protocols, decide how your clinicians evaluate patients and get approved orders, set up a way for your team to reach them, and take part in chart reviews. The physician relationship you need depends on your state, your team’s licenses and the IV services you offer.
Day to day, your medical director helps decide which therapies belong on your menu, which patients are a good fit, who can start and monitor an IV, and what your team should do if a patient has a reaction. These decisions go into physician-reviewed protocols, and they get another look when your menu, staff or locations change.
GuardianMD helps you set up that structure and keep it working. We don’t give infusions or replace the clinical judgment of the people providing care.

Infusion clinic physician oversight
Physician oversight for IV clinics, IV bars, drip bars and IV lounges
Our physician network and tools can support clinics that offer routine IV hydration, vitamin infusions and similar nurse-delivered treatments, as long as the services, prescribing model and staffing are allowed. Planning to add a more complex specialty infusion? That needs its own review of physician qualifications, prescribing, your site’s setup and any extra state rules.
All clinical services are delivered under appropriate physician oversight and in accordance with applicable scope-of-practice requirements.

What GuardianMD does for your IV clinic
Physician match and written roles
We match you with a licensed medical director. If an NP or PA on your team needs one, we also set up a collaborating or supervising physician. Everyone’s responsibilities go in writing.
Treatment protocols and screening
Physician-reviewed protocols that cover who qualifies for treatment, what rules it out, treatment settings and when your team should hold off or escalate care.
Patient exams and orders
A clear process for who evaluates each patient, who can prescribe or give orders, and how it is all documented before the infusion.
Chart reviews and physician questions
Scheduled chart reviews and physician messages, managed through the GuardianMD platform and DocTalk.
Licenses and records
Agreements, protocol versions, messages and review activity are stored in one place, so they are easy to keep track of.
Good Faith Exams for IV hydration
Patient clearance and orders before IV treatment
A Good Faith Exam (GFE) is the medical evaluation a patient gets before treatment. Your IV menu can’t replace that patient-specific check. Your clinic needs an exam and ordering process that fits your patients, your therapies and your state’s laws, plus a plan for new symptoms, reactions or changes in a patient’s condition.
GuardianMD Connect offers telehealth clearance exams for eligible clinics. The exam is done by a licensed NP or PA who works in the same medical practice as your medical director or collaborating physician.
Step 1 · Evaluate
Sees the patient by telehealth
Step 2 · Review
Checks health history and anything that rules out treatment
Step 3 · Order
Writes treatment-specific orders under physician protocols when appropriate
Exams happen by telehealth and are billed separately. Whether a remote exam is right for a certain service depends on your state’s laws and clinical judgment.
Ongoing physician support
Ongoing physician communication and review
When a protocol changes or a clinical question comes up, your team needs a documented way to reach the physician. DocTalk keeps those messages inside the GuardianMD platform, right next to your protocols, agreements and chart review records. That way the relationship keeps working long after onboarding.
Mobile and multi-location IV
Adding mobile IV or new locations
Growth changes your physician setup. Adding an RN, growing your IV menu, offering mobile visits or moving into another state can change the physician’s role, the orders you need, delegation and ownership rules. GuardianMD helps you review those changes and set up any extra physician relationships and paperwork you need.
Operating in more than one state? Our enterprise physician support coordinates your workflows while still following each state’s rules.
Choosing a medical director
How to find a medical director for your IV hydration business
Compare what each option covers, not just the fee. Ask:
Who evaluates patients?
Who issues treatment orders?
How does my team reach the physician?
How are visits documented?
GuardianMD pricing depends on your state, services and staff. There are no setup fees, and physician fees are included in your monthly price, so there is no separate physician bill. We also pay the malpractice insurance for the physician who provides your oversight. Your own team’s malpractice coverage stays with you. Clearance exams are billed separately. Before you add mobile visits or a more complex infusion, review your physician setup first.
