Terms

  1. As an affiliate of Guardian Medical Direction, you will have a dedicated calendar to schedule appointments.
  2. To maintain active status as an affiliate, you must provide at least 30 qualified leads every 90 days. If your status as an affiliate is lost, all payments will be held until you re-qualify.
  3. Payments will be made monthly and require a W9 and submission of ACH information through Bill.com.
  4. By becoming an affiliate, you agree to abide by these terms and conditions and understand that your affiliation with Guardian Medical Direction may be terminated at any time if these terms are not met.
  5. Guardian Medical Direction reserves the right to modify or change these terms and conditions without notice.
  6. By participating in the affiliate program, you acknowledge and agree that you have read and understand these terms and conditions.
