These terms and conditions outline the rules and regulations for the use of our website. By accessing and using our website, you agree to be bound by these terms and conditions. If you disagree with any part of these terms, please refrain from using our website.

Billing Terms:

Setup & Onboarding Fees: A one-time $750.00 set-up fee is due at the time of sign-up. The first month’s subscription fee is also due at the time of sign-up, and the monthly subscription fee will be automatically charged on your billing date.

Account Changes: Guardian Medical Direction bills on a rolling calendar month based on your account creation date (called the “billing date”). Account cancellations, terminations, and other changes must be made ten (10) days prior to the billing date in order for the changes to be reflected on your next invoice. 

No Refunds/Credits: All fees are non-refundable and non-cancellable. Guardian MD does not refund or credit subscription fees for partial months or any portion of a prepaid plan upon deactivation of a Provider or account cancellation. The customer is responsible for all fees (including any monthly minimum) for the entire term of the applicable order or subscription agreement.

  • Must have at least one active licensed provider in the account for the account to remain active.
  • Guardian Medical Direction Platform: Pricing may include cost of additional users

Reactivation Fee: Guardian Medical Direction reserves the right to assess a reactivation fee to Customers whose accounts are suspended based on late payments received more than fifteen (15) days following the payment due date.

Core Plan

  • One (1) Standard Clinical User and one (1) Advanced Clinical User
  • Assignment of Medical Director or Physician Collaborator
  • Physician malpractice insurance
  • Resource library with protocols, consent forms, policy documents, compliance forms, and pharmacy account forms.
  • Correspondence with the Medical Director or Physician Collaborator through the platform
  • Chart reviews. The number of required chart reviews varies by state and licensure but we recommend 10%
  • Includes the most popular services:
    • IV hydration w/ Water soluble Vitamins, Injection of Vitamins (IM), IM Injection of NSAIDs, Lipotropic Injection, Infrared Saunas, Cryotherapy, Red Light Therapy (Low level laser therapy), Chemical Peels, Facials, Skin Care Products, Oxygen Therapy, Event Medicine (First Response, limited to BLS)

Pricing

  • $675/month per account plus one-time $750 set-up fee

Additional users

  • $15/mo per additional Standard Clinical Users (EMT-P or RN license)
  • $500/mo per additional Advanced Clinical Users (NP or PA license)

Elite Plan

  • One (1) Standard Clinical User and one (1) Advanced Clinical User

Pricing

  • $945/month per account plus one-time $750 set-up fee

Additional users

Email and Chat Support

All subscription levels include unlimited access to customer support by email and chat.

Guardian MD Connect

Good Faith Exam Fees: Pursuant to the terms and conditions of the Guardian MD Connect Agreement, the following fee of $39.00 will be billed for each Good Faith Exam.

Practice/MSO Entity Structure Fees

The terms, conditions, and fees for this section shall be governed by and subject to the provisions set forth in the Managed Services Agreement (MSA). Professional Service Fees Professional Services Fees will be determined and quoted in advance of order. Professional services will be billed in advance for the service and any fees incurred will be billed in arrears on a monthly basis.

Fee Changes

All fees may be changed with sixty (60) days’ notice to the Customer. · For all current Customers, Guardian Medical Direction reserves the right to increase its prices by no greater than current US Healthcare Inflation Rate plus 4.0% at any one time, no more frequently than once per 12-month period, upon thirty (30) days’ notice to the Customer.
